LONG-TERM DISABILITY INSURANCE
The Company provides long-term disability insurance at no charge to Corporate and Management employees. This insurance policy will provide income for the employee (at the rate of 60% of gross pay) in case of long-term disability (disability of longer than 90 days). The policy pays from the 91st day on. Disability Insurance Benefits become effective on the 1st of the month following 30 days of employment.

SHORT-TERM DISABILITY INSURANCE
The company provides short-term disability insurance at no charge to Corporate and Management employees. This insurance policy will provide income to the employee (at the rate of 60% of gross pay) in case of short-term disability (disability of longer than 30 days and less than 90 days). The policy pays from the 31st day forward. Disability Insurance Benefits become effective on the 1st of the month following 30 days of employment.

LIFE INSURANCE
$15,000 Life and Accidental Death insurance policy is provided at no charge to Corporate and Management employees.

VOLUNTARY GROUP TERM LIFE INSURANCE
•  Affordable employee coverage up to five times salary in increments of 10,000
   (up to $500,000).
•  Spouse coverage available up to 100% of employee amount in increments of $5,000
   (not to exceed $500,000).

•  Dependent child coverage available (up to $10,000).
•  Guaranteed acceptance amounts for employee, spouse and dependent children.
•  Portability provisions.
•  Accelerated benefit.
•  Premiums deducted every pay period.

HEALTH INSURANCE
The company provides group medical insurance benefits. Medical insurance benefits for Corporate and Management employees becomes effective on the 1st day of the month following 30 days of employment and return of enrollment forms. For hourly employees medical insurance enrollment periods occur January and July following six months of employment.

Each option will be described to you more in detail when you are eligible for insurance. You can decide which plan is best for you at that time.

CAFETERIA PLAN
The company provides a cafeteria plan (Flex Plan) which allows the employee to set aside funds (tax free) into flexible spending accounts to cover charges for out of pocket medical expenses, child care (Flex-Care account), and adoption expenses. Enrollment in the plan is optional.

401K RETIREMENT PLAN
You may enroll in the Company’s retirement plan and contribute to the plan through regular payroll deductions. No income taxes are paid on contributions or earnings in your account until they are withdrawn.

To qualify you must have been with the company for one year, +1,000 hours and you must be 21 years of age. Enrollments are accepted quarterly.

DENTAL INSURANCE
The Company provides group dental insurance benefits as follows:
•  100% - Preventative Services (checkups, cleanings, x-rays, etc.)
•  80%* - Basic Services (fillings, extractions, etc.)
•  50%* - Major Services (bridges, dentures, etc.)
•  50%* - Orthodontics (children under age 19)

* after meeting deductible

Dental benefits become effective on the 1st day of the month following 30 days of employment for management and corporate employees and six months for hourly employees after completion of enrollment forms.

VACATION
Corporate and Management employees are eligible for vacation after completing the following emplooyment duration requirements:
•  Six months of employment - you receive one week paid vacation
•  One year of employment - you receive one week paid vacation
•  Two - four years of employment - you receive two weeks of paid vacation per year
•  Five - nine years of employment - you receive three weeks of paid vacation per year
•  Ten+ years of employment - you receive four weeks of paid vacation per year

HOUSE ACCOUNT CARDS
Managers and Corporate employees enjoy, as a privilege of employment, a House Account for in-store dining. House Account amounts vary depending upon position and length of employment with the company. House Accounts may be used for the purchase of food and non-alcoholic beverages.
